Appointment Information
In the process of booking an appointment and at an appointment itself, we may collect various types of data from you about yourself and your horse(s) in order for us to provide you with our services.
- Contact details (addresses, phone numbers, email addresses etc.) will only ever be used for the purposes of administration, invoicing, sending out of exercise plans and/or reports, and arranging appointments. Such details will not be disseminated to other parties without your permission except where we might perhaps be legally required to do so.
- Details relating to your horse, their history, and the treatment they receive will be used to assist us in providing physiotherapy care for your horse. These treatment records remain the property of Midlands Veterinary Physiotherapy but records may be discussed with your veterinary surgeon where necessary in accordance with the professional requirements of our governing bodies. Please note that details of your horse's historic physiotherapy care might also be requested by your insurance company in the event of you making an insurance claim with them.
- We take the protection of your personal information seriously and have taken suitable and reasonable steps to protect the personal information we hold from misuse, loss, and any unauthorised access, modification or disclosure.
- Treatment records will be stored for a minimum of 7 years as required by our insurers and so you have the right to object to retention of such information at the first point of communication before any treatment commences. Following this minimum period from the last treatment date however, all of your details and records may be securely deleted upon request.
